Öğe Association between depression and eating behaviors among bariatric surgery candidates in a Turkish sample(Springer, 2017) Sevincer, Guzin M.; Konuk, Numan; Ipekcioglu, Derya; Crosby, Ross D.; Cao, Li; Coskun, Halil; Mitchell, James E.Purpose The purpose of this study was to explore further whether depression is associated with problematic eating behaviors in a sample of Turkish bariatric surgery candidates. Methods: This descriptive study included 168 consecutively seen bariatric surgery candidates in a university bariatric surgery outpatient. Participants were asked to complete the Dutch Eating Behavior Questionnaire (DEBQ), the Beck Depression Inventory (BDI) and surveys assessing sociodemographic and clinical variables. Correlations and linear regression analyses were performed to evaluate the relationship between clinical and demographic variables. Results: Participants had a mean age 37.7 +/- 11.3 years and BMI of 46.4 +/- 6.7 kg/m(2) (SD = 6.7). According to BDI scores, 75.5 % of the patients had mild, moderate, or severe depressive symptomatology. Lower levels of depressive symptoms were associated with higher levels of restrictive eating (r = -0.17; p = 0.04), whereas higher levels of depressive symptoms were associated with more frequent eating in response to both internal (r = 0.3; p = 0.002) and external (r = 0.2; p = 0.04) cues. The BDI scores were significantly associated with increased external eating (beta= 0.03, p < 0.02) and emotional eating (beta= 0.03, p < 0.002) scores. BMI (beta = -0.02, p = 0.02 > 0.1) was not associated with DEBQ total scores. Conclusion: This research suggests that mild, moderate or severe depressive symptoms are observed in most of the bariatric surgical candidate patients. There is a positive correlation between severity of depression and emotional/external eating behaviors, and a negative correlation between severity of depression and restrictive eating behavior. Additional research is needed to determine whether treating depression preoperatively can assist with alleviating problematic eating behaviors.Öğe Bariatrik Cerrahinin Psikiyatrik ve Psikososyal Yönleri(Lut Tamam, 2014) Sevinçer, Güzin Mukaddes; Coşkun, Halil; Konuk, Numan; Bozkurt, SüleymanBariatrik cerrahi ülkemizde ve dünyada son on yıl içerisinde giderek popüler hale gelen ve daha çok tercih edilmeye başlayan bir obezite tedavi yöntemidir. Hastaların bariatrik cerrahi ile hızlı kilo vermeleri ve obeziteye komorbid hastalıklarda da düzelmenin beraberinde gelmesi sebebiyle bariatrik cerrahi uygulanan hasta sayısı her geçen gün artmaktadır. Gerek obezite gerekse bariatrik cerrahi psikiyatri ve psikososyal değişkenlerle karşılıklı ilişki içindedir. Bu hastaları değerlendirmede incelenmesi gereken başlıca konular; cerrahi öncesi hastanın herhangi bir psikopatolojisinin var olup olmadığı, cerrahi işlem ve sonrası ile ilgili bilgi düzeyi, cerrahi sonrası ortaya çıkması beklenen fiziksel, psikolojik ve sosyal değişikliklerle ilgili beklenti düzeyleri, cerrahi sonrası ortaya çıkabilecek davranış değişiklikleri ve tıbbi önerilere uyabilme kapasitesinin değerlendirilmesidir. Bu makalede obezite hastalığında bir tedavi yöntemi olarak bariatrik cerrahi hastalarının psikiyatrik ve psikososyal açıdan değerlendirmenin gerekliliği mevcut literatür ışığında gözden geçirilmiştir. Bariatrik cerrahinin muhtemel tıbbi, psikiyatrik ve psikososyal komplikasyonları ve ilgili konular değerlendirilmiş, bariatrik cerrahiden beklenen başarılı sonuç için psikiyatrik bozuklukların ne kadar öngörücü olabildiği ile ilgili literatür derlenmiştir.Öğe Emosyonel Yeme(Türk Psikofarmakoloji Derneği, 2013) Sevinçer, Güzin Mukaddes; Konuk, NumanEmosyonel yeme olumsuz duygulara ikincil aşırı yeme eğilimini anlatan bir kavramdır. Emosyonel yeme ilk başlarda bulimik hastaların aşırı yemelerini destekleyen bir etmen olarak emosyonların yer aldığı bir tablo içinde bulimia ile birlikte anılmıştır. Sonrasında ise tıkınırcasına yeme ataklarının da emosyonel yeme ile ilişkili olabileceği gösterilmiştir. Olumsuz duygular karşısında gözlenen bu aşırı yeme tepkisi; obez bireylerde, yeme bozukluğu olan kadınlarda ve normal kilolu olup da diyet yapan kişilerde mevcut olduğu gösterilmiştir. Özellikle kilo verme girişimlerinde yeme davranışlarının düzenlenmesinde emosyonel yemenin önemi giderek artmaktadır. Psikiyatrik bozukluklarda gözlenen yeme ve kilo değişiklikleri, ilaç yan etkileri ile ilişkili ya da ilişkisisiz metabolik sendrom ve diabet, ve kilo verdirici tedavilerle iç içe geçen psikososyal konular da dikkate alındığında emosyonel yeme kavramının daha kapsamlı araştırılması gerekliliği aşikardır.Öğe Food addiction and the outcome of bariatric surgery at 1-year: Prospective observational study(Elsevier Ireland Ltd, 2016) Sevincer, Guzin Mukaddes; Konuk, Numan; Bozkurt, Suleyman; Cokun, HalilPurposes: The objectives of the current study were to determine the prevalence of food addiction in bariatric surgery candidates and whether food addiction is associated with weight loss after bariatric surgery. Methods: This prospective observational study was performed on morbidly obese patients who had been found suitable for bariatric surgery. Follow-up was conducted at the 6 and 12 month post-surgery. The Yale Food Addiction Scale (YFAS) was used to determine food addiction. Results: One hundred seventy-eight patients were included. Pre-operative food addiction was found in 57.8% of patients. Food addiction prevalence decreased at the 6 and 12 month follow ups, to 72% and 13.7% respectively. There were no statistically differences between those with food addiction and those without addiction with regard to weight loss measured as percent of excess BMI at the 12 month follow-up. Conclusion: Food addiction as measured by the YFAS decreases significantly after bariatric surgery. The presence of food addiction before surgery was not associated with weight loss as measured EBL%. However, in view of the increase in BMI, 2 years after surgery longer follow up studies are warranted. (C) 2016 Elsevier Ireland Ltd. Methods: Eight hundred and twenty-six (women: 532; men: 294) university students volunteered and took part in the study. Yale Food Addiction Scale (YFAS), Adult ADHD Self-Report Scale (ASRS-v1.1), Barratt Impulsiveness Scale-11 (BIS-11) scales were applied to the participants and their anthropometric measurements were taken. Analyses were conducted using SPSS version 22. Results: 109 subjects were classified as food addicts. The food addict group significantly had higher Body Mass Index (BMI) (p <0.001). The subscore of inattentiveness, subscore of hyperactive/impulsiveness and total ASRS scores were significantly higher in food addicted group (p <0.001). BMI was greater in the food addiction and ADHD comorbidity group compared to the food addiction only group (X2=24.86; p=0.003). The lineer regression analysis revealed that the increased hyperactivity – impulsiveness measured by ASRS (ß=0,027), motor impulsiveness measured by BIS-11 (ß=0.030) and BMI (ß=0.037) found to be correlated with higher YFAS scores (F=10.384; p=0.000) when age and sex factors are controlled.) YFAS scores were statistically higher in potential ADHD group. (According to the ASRS cut-off score ?24). Conclusion: Evaluation of the presence of ADHD might be an important issue in determining treatment for obesity in individuals with food addiction. Olga; Akdemir, Asena; Sevinçer, Güzin Mukaddes; Atik, LeventAmaç: Bu çalışmanın amacı, psikiyatrik ayaktan hasta popülasyonunda Gece Yeme Sendromunun (GYS) şiddetini ölçmek için kullanılan Gece Yeme Anketinin (GYA) Türkçe sürümünün geçerlilik ve güvenilirliğini değerlendirmektir. Yöntem: GYA’nın Türkçe sürümünün faktör yapısı, iç tutarlılığı ve geçerliliği incelenmiştir. İlk olarak GYA’nın Türkçeye çevirisinin ardından İngilizceye tekrar çeviri yapılmıştır. Düzeltme ve semantik uyarlama sonrası anketin anlaşılabilirliği değerlendirilmiştir. GYS tanısı psikiyatrik görüşme yapılarak konulmuştur. GYA 433 hastaya, iki hafta sonra yapılan tekrar testi ise 141 hastaya uygulanmıştır. Sonuçlar: Cronbach alfa katsayısı 0.69 bulunan anket yeterli iç tutarlılık göstermektedir. Test-tekrar test toplam puanları arasındaki intraclass korelasyon katsayısı 0.96 olarak bulunmuştur. Psikiyatrik değerlendirme sonucunda 97 hastada GYS saptanmıştır. ROC eğrisi altında kalan alan 0.90 olarak bulunmuştur. GYA puanında 18 değeri sınır değer olarak kabul edildiğinde duyarlılık %90.7, seçicilik %73.8 olarak saptanmıştır. GYA’nın doğrulayıcı faktör analizinde orijinal ankete benzer şekilde dört faktörlü yapı elde edilmiştir. Tartışma: Bu çalışmaya göre GYA’nın Türkçe sürümünün geçerli bir test olduğu, iyi tanısal özellik gösterdiği ve iç tutarlılığının yüksek olduğu söylenebilir. Anahtar Kelimeler: Gece Yeme Sendromu, Gece Yeme Anketi, geçerlilik, güvenilirlik Objective: The purpose of this study was to evaluate the reliability and validity of Turkish version of the Night Eating Questionnaire (NEQ) in psychiatric outpatient population as a measure of severity of the Night Eating Syndrome (NES). Methods: We examined the factor structure, internal consistency and validity of the NEQ in Turkish. First, we translated NEQ into Turkish; and then back-translation into English; correction and semantic adaptation and assessment of the understanding of the questionnaire. We made the diagnosis of NES according to psychiatric examination. We administered the NEQ in 433 patients and the re-test in 141 participants two weeks later. Result: The instrument showed satisfactory internal consistency with an overall Cronbach α of 0.69. Intraclass correlation for the test re-test total score was 0.96. The ninety seven patients had NES with psychiatric examination. The area under ROC curve defined as 0,904. For cut-off point of 18 in NEQ both sensitivity and specificity were 90.7% and 73.8%. The confirmatory factor analysis of the NEQ yielded a similar four factor solution as the original scale. Conclusions: We found that Turkish version of NEQ is valid and has well diagnostically performance, and high internal consistency. Methods: Two hundred and ten students (99 men and 111 women) were included. All participants completed a sociodemographic form, Barratt Impulsiveness Scale (BIS), Beck Depression Inventory, Beck Anxiety Inventory, Night Eating Questionnaire (NEQ) and Eating Disorder Examination - Questionnaire (EDEQ). Body mass index (BMI) of the participants were measured. Results: We found 9.5% of the participants screened positive for NES. Overall scores for NEQ showed significant positive correlation with depression and anxiety symptoms as well as total score and sub-scores of BIS and EDE-Q. Predictive values of depression, anxiety and impulsivity were found to be higher than that of EDE-Q for NES. Conclusion: NES is prevalent in university students. Depression and anxiety symptoms might predict NES. Impulsivity might be of significance for NES psychopathology as well. Impulsivity is more likely to predict NES when compared to EDE-Q scores.Öğe The prevalence and clinical features of the night eating syndrome in psychiatric out-patient population(W B Saunders Co-Elsevier Inc, 2015) Saracli, Ozge; Atasoy, Nuray; Akdemir, Asena; Guriz, Olga; Konuk, Numan; Sevincer, Guzin Mukaddes; Ankarali, HandanObjective: In this study we aimed to investigate the prevalance and clinical correlations of night eating syndrome (NES) in a sample of psychiatric outpatients. Method: Four hundred thirthy three consecutive psychiatric out-patients older than 18 years were evaluated in the outpatient clinics using clinical interview according to the DSM-IV with regard to psychiatric diagnosis. Participants were also screened for presence of NES utilizing both clinical interview and self report based on Night Eating Questionnaire (NEQ) instruments. Sociodemographic and clinical features such as age, gender, education level, socioeconomic level and body mass index (BMI) were also recorded. The Body Shape Questionnaire (BSQ) and the Symptom Checklist-90 Revised (SCL-90R) were administered. Results: Based on the proposed diagnostic criteria of the NES via utilizing clinical interview method, 97 (32 male, 65 female) of the sample met diagnostic criteria for NES. The point prevalence of NES was 22.4%. No statistically significant differences were found between the two groups in terms of age, gender, marital status, education and BMI. The patients with NES had higher NEQ, BSQ and SCL-90R subscale scores than patients without NES. Prevalance of depressive disorder, impulse control disorder, and nicotine dependency was higher among patients with NES. No differences were found with regard to the medication (antipsychotics, antidepressants and mood stabilizers). Conclusion: Night eating syndrome is prevalent among psychiatric outpatients and associated with depression, impulse control disorder, and nicotine dependency. Body dissatisfaction and higher symptom severity are also other risk factors for the development of NES. (C) 2014 Elsevier Inc. Yöntem: Bariatrik cerrahi arayışında olan obez hastalara (n=171) Yale Yeme Bağımlılığı Ölçeği uygulandı. Ölçeğin yapı geçerliliği faktör analizi ile, güvenilirliği madde toplam puan korelasyonu ile, tekrarlanabilirliği ise test-tekrar test sonuçları arasında sınıf içi korelasyon katsayısı hesaplanılarak değerlendirildi. Bulgular: Yirmi beş maddeli ölçeğin Kuder-Richardson Formula-20 katsayısı ile hesaplanan iç tutarlılığı (KR-20=0.822) ve Cronbach alfa (0.859) değerleri yeterli bulundu. Bartlett's Sphericity Testine göre geliştirilen faktör yapısının anlamlı olduğu kararına varıldı. Faktör analizine göre altı belirgin faktör olarak kümelenen bu yapı toplam varyansın %67.51’ini açıklıyordu. Ölçeğin madde toplam korelasyonları 0.214-0.666 arasında idi. Sonuç: Bulgularımız Yale Yeme Bağımlılığı Ölçeğinin Türkçe formunun bariatrik cerrahi hasta grubunda yeme bağımlılığını ölçmede güvenilir bir araç olduğunu göstermektedir.Öğe The relationship between the premorbid personality traits and the behavioral and psychological symptoms of Alzheimer disease(Elsevier GmbH, 2017) Sevinçer, Güzin Mukaddes; İpekçioglu, Derya Yalçın; Konuk, Numan; Ertan, TuranAim In this study, we have aimed to investigate the relationship between the premorbid personality traits and the behavioral and psychological symptoms of Alzheimer disease. Method 147 dementia patients who have attended to Geriatric Psychiatry Department of Istanbul University Cerrahpaşa Medical Faculty have been selected randomly. Out of these patients, 45 Alzheimer type dementia patients whose family members or a relative could be reached are included in the study. Relatives of each patient have been asked to fill in the Munich Personality Test (MPT) – relatives form for the purpose of assessment of premorbid personality traits of the subjects. Results It could be stated that high level of premorbid isolation tendency and schizoidia scores are the predictors of hallucinations and aggression. Low level of premorbid frustration tolerance is found to be associated with the present anxiety and the higher Behave–AD general assessment scores. Conclusion We conclude that certain premorbid personality traits could be a risk factor for the later development of behavioral and psychological symptoms in Alzheimer's disease or that psychopathologic symptoms could actually be exaggerations or morbid distortions of lifelong behavioral tendencies, and it may also be explained by reflection of neuropathological processes occurring in brain in dementia onto clinic practice, in reliance upon detection of similar findings in studies conducted on different types of dementia and in different cultures. Advanced studies conducted on a sufficient number of subjects in prospective design within a biopsychosociocultural model are needed. © 2017
