Bilgisayarlı dinamik postürografi değerlerinin vücut kitle indekslerine, cinsiyet ve yaş gruplarına göre değişimi
Özet
Denge sistemi çok biyomekanik süreçleri içinde barındıran karmaşık bir mekanizmadır. Bilgisayarlı Dinamik Postürografi (BDP) cihazı vücudun dengesini değerlendiren altın standartta bir test olup, günümüzde kliniklerde yaygın olarak kullanılmaktadır. BDP her yaş, cinsiyet, boy ve kilodan bireyler için uygulanmaktadır. Ülkemizde BDP ile yapılan klinik çalışma sayısı az olduğundan araştırmamız normatif veriye de katkı sağlayacaktır. Bu çalışmanın amacı, yaş, cinsiyet ve beden kitle indeksinin denge üzerine etkisini incelemek, denge sisteminin değerlendirilmesinde kullanılan Bilgisayarlı Dinamik Postürografi üzerinde etkisinin olup, olmadığını araştırmaktır. Çalışma Özel Acıbadem Kozyatağı Hastanesi, Odyoloji Kliniği'nde gerçekleştirilmiş olup, işitme ve denge ile ilgili şikayeti olmayan 20-60 yaş arasında 70 gönüllü sağlıklı bireylerle yürütülmüştür. Verilerin analizinde SPSS 28.0 programı kullanılmış, hastaların denge puanları değişkenlerle birlikte ortalama ve standart tablosu olarak verilmiştir. Yaş, cinsiyet ve beden kitle indeksi grupları ile denge parametrelerinin ilişkisini belirlemek amacıyla Ki-Kare testi (ChiSquare test) kullanılmıştır. Ki-kare istatistiğinde ''p'' değeri karşılaştırmada anlamlı farkı gösterir ve 0,01 ile 0,05 arasında yer alır. Çalışmamızda p < 0,05 anlamlı kabul edilmiştir. Erkek ve kadınların denge puanları kıyaslandığında anlamlı fark bulunmadı. Yaşa göre bakıldığında; SOM/AP, VİS/AP, PREF/AP değerleri anlamlı (p>0.05) farklılık göstermemiştir. Yaş≥38 olan grupta VEST/AP ve GLOB/AP değerleri yaş≤37 olan gruptan anlamlı (p<0.05) olarak daha düşüktü. Yaş≤37 ve yaş≥38 olan gruplar arasında SOM/ML ve PREF/ ML değeri anlamlı (p>0.05) farklılık göstermemiştir. Yaş≥38 olan grupta VİS/ ML, VEST/ML ve GLOB/ML değerleri, yaş≤37 olan gruptan anlamlı (p<0.05) olarak daha düşüktü. Vücut kitle indeksine göre değerlendirilen denge puanlarında AP düzlemde gerçekle- şen test durumlarında VKİ<25 ve VKİ≥25 olan gruplar arasında SOM/AP, VİS/AP, PREF/AP, VEST/AP, GLOB/AP değerleri anlamlı (p>0.05) farklılık göstermemiştir. VKİ<25 ve VKİ≥25 olan gruplar arasında SOM/ ML, VEST/ML ve GLOB/ML değerleri anlamlı (p>0.05) farklılık göstermemiştir. VKİ<25 olan grupta VİS/ML değe- ri VKİ ≥ 25 olan gruptan anlamlı (p<0.05) olarak daha düşüktü. VKİ<25 olan grupta PREF/ ML değeri VKİ≥ 25 olan gruptan anlamlı (p<0.05) olarak daha düşüktü. Çalışmamızda tüm katılımcı verilerinin olduğu genel korelasyon analizi sonucunda, artan yaşla birlikte Global ve Vestibüler denge skorlarının azaldığı görülmüştür. The balance system is a complex mechanism that includes many biomechanical processes. The Computerized Dynamic Posturography (BDP) device is a gold standard test that evaluates the balance of the body and is widely used in clinics today. BDP is applied for individuals of all ages, genders, heights and weights. Since the number of clinical studies conducted with BDP in our country is low, our research will also contribute to the normative data.The aim of this study is to examine the effect of age, gender and body mass index on balance and to investigate whether it has an effect on Computerized Dynamic Posturography used in the evaluation of balance system.The study was carried out in the Audiology Clinic of Private Acıbadem Kozyatağı Hospital, and it was carried out with 70 healthy volunteers between the ages of 20-60 with out ear and vestibular system complaints. The SPSS 28.0 program wasused in the analysis of the data, and the balances- cores of the patients were given as the mean and standardt able with the variables. Chi-Square test (ChiSquare test) was used to determine the relation ship between age, gender, body mass index groups and balance parameters. The "p" value indicates a statistically significant difference in a comparison and takes a value between 0 and 1 in the Chi-Square istatistic. The closer the value is to 1, the stronger the relationship. If the "p" value is between 0.01 and 0.05, there is a significant difference. When the balance scores of men and women were compared, no significant difference was found.According to age; SOM/AP, VIS/AP, PREF/AP values did not differ signifi- cantly (p>0.05) VEST/AP and GLOB/AP values were significantly (p<0.05) lower in the age ≥ 38 group than in the age ≤37 group. SOM/ML and PREF/ML values did not differ significantly (p>0.05) between groups aged≤37 years and ≥38 years. The VIS/ML, VEST/ML and GLOB/ML values in the age≥38 group were significantly (p<0.05) lower than the age≤37 group. SOM/AP, VIS/AP, PREF/AP, VEST/AP, GLOB/AP values did not differ significantly (p>0.05) between groups with BMI<25 and BMI≥25 in balance scores evaluated according to body mass index. SOM/ML, VEST/ML and GLOB/ML values did not differ significantly (p>0.05) between gro- ups with BMI<25 and BMI≥25. In the group with BMI<25, the VIS/ML value was significantly (p<0.05) lower than the group with BMI≥25. The PREF/ML value in the group with BMI<25 was significantly (p<0.05) lower than the group with BMI≥25. As a result of the general correlation analysis of all participant data in our study; Global and Vestibular balance scores were found to decrease with increasing age.
